All regular, full-time Leighton employees become eligible for benefits on the first day of the month following 30 days from date of hire. LIFE & ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE (Insured through Mutual of Omaha)Benefit: 1X annual earnings to $300,000 Cost: Paid in full by the Company. You may also purchase additional life insurance for yourself and your dependents through Mutual of Omaha. GROUP MEDICAL INSURANCE (Insured through Blue Shield) We offer an HMO (Health Maintenance Organization) and POS (Point of Service) plan. Choose the plan that best suits your needs. The HMO plan requires that you seek all services within the network. The POS option lets you choose where you want to receive your care.
GROUP DENTAL INSURANCE (Insured through Golden West) Each year, you may select either the Prepaid or PPO (Preferred Provider Organization) plan. Under the Prepaid plan, you must use Golden West providers to obtain benefits. Under the PPO, you may use contracting or non-contracting dentists. However, you will receive higher benefits through a prepaid dentist. DISABILITY INSURANCE (Insured through Mutual of Omaha) Leighton provides you disability insurance at no cost to you. You may purchase additional disability benefits to enrich your benefits. EMPLOYER CONTRIBUTIONS Leighton will share in the cost of medical insurance for employees and their families. Leighton pays the full cost of the pre-paid dental plan for employees and their families. You may buy up to the PPO plan if so desired.
